First block the connection by calling customer care number 1800 266 1800 or 1860 266 5555 from other network or by contacting their online customer care chat support.
Step to follow to block sim:
1. Go to their online website and click on following link
2. Then click on “Chat support” link at bottom of the page.
3. Talk to TATA DoCoMo support and give him details required.
If all the proofs and details given by you are correct, he will proceed to block the SIM.

For new replacement sim:
You can also get new sim on the same number by visiting nearest Tata DoCoMo store,
- Fill duplicate sim form,
- Provide one photograph,
- Photo ID & signature proof and proof of address.
- SIM replacement charges
New SIM will get activated within 3 hrs post submitting request.
